Job Description

Position Summary
Demonstrates the knowledge and skills necessary to manage care of the cardiovascular patient in ambulatory or hospital settings.  Works in collaboration with physicians and other members of the interdisciplinary team to evaluate and treat, order testing or additional services, conduct and interpret appropriate diagnostic and laboratory tests, and prescribes pharmacologic agents and treatments in accordance with established protocols, policies and evidenced based practice.  Provides expert education, consultation, and patient advocacy.

Primary Job Responsibilities and Accountabilities
•    Assesses condition of the patient based on physical examination, results of laboratory and radiologic tests and other relevant data and develops a patient care plan. Monitors data and alters plan of care accordingly.
•    Provides high quality acute care and extended management of cardiovascular patients in an ambulatory or inpatient setting, including ordering and evaluating appropriate diagnostic tests, collaborating with health care team members, and treating complications.
•    Initiates, directs, and supervises resuscitation measures on the patient during emergency situations until emergency medical transport or physician arrives. 
•    Communicates with other patient care providers including physicians, other advanced practice providers, residents, medical students, RNs, etc to exchange information on condition of the patient, discuss progress (or lack of progress) and serves as resource to the same regarding care of the patient.
•    Communicates with patients and their families results of tests performed, progression towards goals, and educates regarding signs and symptoms of disease management. Continually re-evaluates plan of care with patient/family based on success of interventions and degree of patient satisfaction. 
•    Educates care givers and significant others/family regarding care of the patient and provides emotional support and guidance to the same.
•    Completes all necessary documentation. Maintains full and current medical records of patients.
•    Communicates with referring providers to inform them of results of initial evaluation as well as continued follow-up on patient’s progression with plan of care.
•    May supervise cardiac nuclear stress tests.
•    Advises or suggest to Director of Physician Practices or designee in the selection, maintenance and utilization of equipment in OSF facilities.
•    May develop and present educational programs on the care of the cardiovascular patient to other nursing and healthcare providers within OSF HealthCare as well as community outreach opportunities.
•    Exercising diligence in assisting the Ministry in keeping costs of the Practice to a minimum while maintaining a high level of quality care for patients.
•    Providing other services as reasonably requested.

Basic Qualifications

Required
•    Current Certification as APN or Physician’s Assistant 
•    Current Licensure in the state of Illinois as an APN or Physician’s Assistant 
•    Licensed by the Federal Drug Enforcement Administration and State of Illinois for prescribing of narcotics, if applicable
•    Current Basic Life Support certification and Advanced Cardiac Life Support certification

Preferred
•    Two years of cardiovascular patient care experience.
•    Two years of critical care patient management experience

Other skills/knowledge

•    Work requires the interpersonal skills necessary to educate family and significant others on care as well as provide emotional support, deal effectively with providers, residents, medical students, and hospital or office staff, provide training and education to professional and community.
