Hope PACE in Fort Myers, FL is seeking a Primary Care Provider (Geriatrics, Internal Medicine or Family Medicine) with a strong interest in Geriatric Medicine. The Program for All-Inclusive Care for the Elderly (PACE) provides comprehensive medical and social services to older patients by integrating the patient’s goals and values, comorbidities, and prognosis into the practice of evidence based medicine. A Physician who is looking for an opportunity to build longitudinal relationships and drive appropriate care for each participant will thrive in this position. Equally important is a Physician with exceptional communication and documentation skills!
Hope PACE Opportunity
- Join a collegial interdisciplinary care team.
- Full-time position – Monday – Friday. Center hours are 8:00 am – 4:30 pm
- Participants are seen by the Primary Care Provider between 10:00 am – 2:00 pm.
- Dedicated administrative time between 2:00 pm – 4:30 pm.
- The average number of participants seen per day is 3 – 8.
- No Call responsibilities. Call is taken by a Call Team.
