Job Description

Multi-specialty health care group in Tulsa, OK is seeking an acute care nurse practitioner for palliative care role, joining an established group that includes one physician, three advanced practice providers and a social worker. This practice has a tenured support staff and will be located mainly at the Medical Center but will have rotations at a second location. This full time opportunity will cover Monday through Friday 8am-5pm with no weekend call obligations. Acute care nurse practitioner or a PA. Assist patients and families with goals of care discussions, code status, advance directives and education about hospice/palliative services Assist with GIP admissions and follow up Facilitate hospice and home palliative care disposition planning Promote awareness and education for nursing and medical staff Facilitate the early identification of possible appropriate palliative care inpatients Assist with monitoring symptom management for inpatient palliative care patients Provide administrative assistance to the palliative care medical director Hospice and palliative care experience is preferred Compensation andamp; Benefits: Compensation and benefits are competitive.
