Family Nurse Practitioner options located in Oregon’s scenic Willamette Valley
Family-NP - Nurse Practitioner - Annual Wellness Visit Program
The clinic is actively recruiting a certified family nurse practitioner or certified adult geriatric nurse practitioner to develop and implement our CMS Annual Wellness Visit Program. The nurse practitioner will perform the annual visits or facilitate their completion with the patients' primary care provider. The goal is to increase the number of completed wellness visits while maintaining the highest level of patient care.
Responsibilities:
Complete a comprehensive patient and family assessment to determine priority needs and, in collaboration with the patient's physician and other team members, design and implement an individualized plan of care
Promote education of medical health conditions. Ensure that the patient and|or their caregiver are educated on managing their medical condition and comorbidities to help them attain individualized care plans with the goal of improving medical outcomes
Use multiple interventional strategies and opportunities to assist patients with self-care and symptom management
Collaborate with operations directors and managers to schedule annual wellness visits at multiple sites and occasionally in patients' homes
Qualifications:
ANCC or AANP Certification
Minimum one year experience working as a licensed nurse practitioner; geriatric experience preferred
Experience with CMS compliant AWV's highly desirable
Excellent written and oral communication and problem solving skills
Strong customer service orientation
Oregon License or ability to obtain an Oregon license
Valid Oregon driver's license and reliable transportation
Family-NP - Nurse Practitioner for Immediate Care Center in the Albany, OR area
Our Clinic, is actively recruiting a certified Family Nurse Practitioner to join a busy Immediate Care Center
Opportunity:
15 - 16 shifts per month or approximately 160 hours
Work shifts are 10-12 hours during the week; 5-7 hours, weekends and holiday shifts
2 Providers per shift; 40-70 patients per shift
Full diagnostic back-up available, adjacent to Level II Trauma Center
