Sanford Federal, Inc. is seeking a Board Certified Otolaryngologist to provide clinical Otolaryngology services, including, but not limited to: Clinic and Surgical Care, Operative Services, Intraoperative Follow-up, Administrative duties, Consultative services, Medications and Discharge education.

Job Type:
AS NEEDED every Tuesday and Friday between 07:30 AM and 4:00 PM, for clinics, emergent services, and after-hours consultations.
-Tuesday and Friday Mornings will be used for surgery and afternoons for clinic.
-Emergent service (emergent medical conditions): approximately 10 hours annually.
-After-hour phone consults (outside normal VAMC clinic hours): approximately 80 hours annually.
Responsibilities:
· Be present on time for any scheduled clinics/surgeries in the clinic or operating room at the scheduled start time.
· Typical surgical procedures include, but are not limited to: Sinus Surgery, Head/Neck cancer removal, Panendoscopy, Tonsillectomy, Septoplasty, Rhinoplasty, and Laryngoscopy.
· Provide consultative services at the patient’s bedside if the patient is not ambulatory and in the clinic setting if the patient is able to report to the outpatient clinic.
· Follow all established medication policies and procedures.
· Provide discharge education and follow up instructions that are coordinated with the next care setting for all Otolaryngology clinical or surgical patients.
· Respond to the VAHCS within 1 hour for any Emergent Services.
Qualifications:
· License to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia). All licenses shall be full and unrestricted licenses in one or more states, but whoever have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered.
· Provide proof of the following tests:
- 1/ TUBERCULOSIS TESTING: Tuberculosis Skin Test (TST) or interferon-gamma release assays (IGRA) A negative chest radiographic report for active tuberculosis shall be provided in cases of positive TST or IGRA results.
- 2/ MEASLES, MUMPS, & RUBELLA TESTING.
- 3/ VARICELLA.
- 4/ ACELLULAR PERTUSSIS: Provide proof of 1 dose of Tdap vaccination
- 5/ INFLUENZA: Provide proof of the annual Influenza vaccine.( In case of a medical contraindication to the vaccine, wearing a mask is required during the Influenza season).
- 6/ Provide evidence of completing and passing generic self-study blood-borne pathogen training.
{This is applicable to all health care workers}; provide their own Hepatitis B vaccination series and hepatitis B surface antigen test results following the hepatitis B vaccination series; maintain an exposure determination and control plan; maintain required records; and ensure that proper follow-up evaluation is provided following an exposure incident.
· Must have/ obtain appropriate NPI (National Provider Identifier).
· Provide copy of current DEA certificate.
· Board Certified by the American Board of Otolaryngology and be currently certified in Basic Life Support (BLS) Advanced Cardiac Life Support (ACLS) or equivalency.
· Provide verifiable evidence of all educational and training experiences including any gaps in educational history.
· Provide copies of current CMEs.
