Hospitalist scope of work

- Accepting locums, permanent, and locums to permanent

- Minimum 3-month assignment starting as soon as possible, ideally 6 months

- Board certification required, or board eligible within 5 years of residency

- Must be IM or FP trained (U.S. residency)

- Hospitalist experience preferred but not required

- Malpractice coverage is provided under the Tort Claims act

- ACLS/BLS required, hospitalists expected to run codes on their own patients

- Procedures not required (encouraged with proctoring available if requested)

- Average of 9-12 patients per shift with average of 1-2 admissions per shift

- Open 4-bed ICU (no intensivist available, must be comfortable with basic ventilator management, other team members available for backup).

We are a remote island, and the only hospital and ICU. Our patients have no other facility in general to divert to, no medivac service to a larger hospital.  We are it.  Our acuity runs from minor to catastrophic, but not so much major trauma. The interventions available are less than on the mainland, but the patients equally sick.  We offer backup and hand-holding, but this is not a Wallmart store front operation. It is not for the timid.

- Generally 3 hospitalists on day shift (a total of 6 hospitalists on the team)

- Minimal specialty backup

       (full-time: general surgery, nephrology, anesthesia, and OB-GYN)

       (part-time:  GI 2-wks/mo, IR 1-day/mo)

- EHR is RPMS (essentially the same as VistA at VA hospitals)

- Schedule:

  • Two week blocks on a continually rotating basis
    • First week is Monday - Sunday 8 am to 4 pm
    • Second week 4 days off and 3 consecutive days of night call
      • Night call is from home 4 pm to 8 am
        • Occasional phone calls
        • Required to go to the hospital to see patients and place an H&P on the chart from 4 pm to 10 pm
        • After 10 pm, the ED “tucks in” patients and they are seen by the dayshift hospitalists
        • Required to go to hospital at any time of night for seriously deteriorating patient (rare) and for all ICU admissions
  •  Usually reasonable patient volume, average of 9-12 inpatients inclusive of consults
  • Length of hospital stay tends to be longer on the island as there are no nursing homes so there is a more consistent relationship with patients (fewer H&Ps and discharges, more daily management)
  • For locums and other new staff, a backup physician is designated who is available to answer any questions or present for assistance as may be necessary at any time.

Salary is $218K currently for hospitalists. With our tax structure here it is closer to $372K.   My belief is this is the best place for service and adventure aspect of this position.  We are looking for another hospitalist to join our group of  6 that will be going down to 5 at the end of march.    Typically the hospitalists work 7 days in a row (typically 8-5) followed by 3 nights the next week. A "night" means taking all admissions until 10pm and any calls and ICU admissions after 10.   Non ICU admissions 10-8 are given hold over orders by the ER until the morning shift comes in. 

Negligible fed taxes. Max 9.5% for our combined local and federal tax so makes take home pay much higher. We are the only hospital for the island chain, 86 bed hospital with 4 bed ICU.With 4 MD's for instance, we have 2 MD's in the hospital during the days for 7 days (Mon.- Sun. with 8Am to 4Pm shifts) and the other MD's split the night coverage that week with 4Pm-8AM shifts. (They switch roles the following week.) The Hospital has a 4 bed ICU, NICU and a Dialysis Unit for 100 renal failure patients plus state of the art diagnostic facilities. Canadians:

The bottom line is that if the Canadian physician has passed USMLE, steps I, II and III, and has an offer of employment in the U.S., it is a simple procedure to obtain H-1B status to practice medicine in the U.S. However, most Canadian medical graduates take the LMCC although a small minority takes a U.S. examination. Few of these physicians relish the prospect of having to interrupt their careers in order to take and pass the USMLE.

Is temporary TradeNAFTA (“TN”) work status a viable alternative to H-1B status for Canadian physicians? Unfortunately, the answer is no. TN physicians may enter the U.S. to teach or do research only, not to practice medicine.
