• Full-time Nurse Practitioner position in the Chelsea area of NYC. • Work with homeless patients mostly single adult men. • Services include mental health care, employment support, substance abuse programs, and housing assistance (transitional and permanent). • Collaborate closely with a dynamic head nurse and team of clinicians to connect residents to needed services. • Role focuses on evaluations and care coordination rather than heavy medication management. • Future opportunity to manage a small caseload for meds and therapy. • Schedule: Three long days per week (option for a four-day workweek if preferred).
