GENERAL SURGEON

Salary $270K  (the CNMI enjoys one of the lowest tax burdens in the U.S.; please note the tax information provided below)

Qualifications:

• Permanent much preferred, locums possible

• Board certification or board eligible within 5 years of residency required

• Must have completed U.S. or Canadian residency program and be fully authorized to work in the U.S.

• Must be Medicare reimbursement eligible

• ACLS/BLS required

• CNMI license required (application process supported by HR staff)

Licensing and credentialing takes 6-8 weeks (Process: Present CV and, if cleared, the candidate will be notified by email with employment application attached. Three recent references are also required.)

The local specialty backup available includes:

• Full-time: Internal Medicine; general surgery; orthopedic surgery; oncology;  nephrology; anesthesia; OB-GYN; psychiatry; podiatry; and soon to add, diagnostic radiology

• Part-time:  Interventional radiology 1-day/mo; 

  • Currently routine radiology is provided by telemedicine from Guam

  • Telemedicine assistance for Neurology and Dermatology

Schedule: The current schedule is as follows:

• Monday to Friday, split surgery admissions, round on own patients. 

• Two days night call taken from home unless requiring urgent evaluation or surgery, followed by 2 days off, and alternating weekends (Friday PM to Monday AM).

• Emergency surgeries are typically infrequent.

• $10,000 relocation assistance for new, off-island hires signing an initial contract of at least 2-years.

• International flight from the point of recruitment for the employee.  Return flight to original embarkation is included and will be arranged upon completion of contract.

• Malpractice coverage is provided under the CNMI Government Liability Act (essentially identical to the Federal Tort Claims Act).

• Reimbursement of up to $250 for pre-employment health screening costs.

• Holidays:  The CNMI currently observes 5 state and 9 federal holidays for a total of 14 paid holidays.

• Annual Leave: Accrues at eight (8) hours per pay period (bi-weekly), with a full-time Employment Contract

• Sick Leave: Accrues at four (4) hours per pay period (bi-weekly)

• Up to 5 days of paid leave for CME.

One of the lowest tax burdens in the U.S.: 

• • If in the CNMI 183 days or more during the tax year (and otherwise meets requirements), then CNMI residency status is met for that year. As a result, earnings are taxed at the maximum CNMI (“state”) tax rate of 9% and there is a 90% rebate of U.S. federal income tax on income earned within the CNMI. 

  • If in the CNMI less than 183 days, the IRS taxes the income earned within the CNMI the same as U.S. mainland earnings for federal income tax purposes. 

  • For state taxes, money earned within the CNMI has a maximum “state” tax of 9%.   Money earned outside of the CNMI is paid at that state’s tax rate.  

  • There is no sales tax in the CNMI.

What this means for residents is that we do not file with the U.S. IRS, but only with Tax and Revenue here on island.  After-tax amount retained on a $270,000 salary here is roughly equivalent to that of a $410,000 salary in New York or San Francisco.  Add to that the fact that there is no sales tax in the CNMI!
