What you'll be doing
- Comprehensive outpatient primary care for a diverse adult patient population in Upland.
- Proactively address HEDIS and STARs measures to improve health plan quality ratings.
- High-risk patient huddles and care coordination with clinical and support teams.
- Collaborate with clinic leadership on productivity, efficiency, and performance metrics.
- Manage medically complex patients with multiple chronic conditions through a proactive, preventive care model.
- Focus on reducing avoidable ER visits and hospitalizations while improving quality outcomes.
- Utilize Athenahealth (Athena) EHR to support coordinated, patient-centered care.
Compensation & benefits
- $232,506–$348,758 base salary
- Individual and company performance bonus

What you bring
- MD or DO degree; board certified — or board eligible, must certify within 12 months
- Current, unrestricted CA medical license and valid DEA number
- STARs, HEDIS, or Medicare Advantage quality experience a strong plus
- TB test required (CareMore Health business unit requirement)
