Job Description
Position Purpose & Focus
The SharpCare Nurse Practitioner (NP) or Physician Assistant (PA) is responsible for conducting comprehensive patient assessments, diagnosing and managing acute and chronic conditions, and implementing evidence-based treatment plans across the lifespan, consistent with their training and the needs of the clinic. The primary objective of the NP/PA role at SharpCare is to provide high-quality, patient-centered care with a high degree of clinical autonomy within the scope of practice. This role fosters patient trust through education and compassionate care while supporting a collaborative, team-based approach to healthcare delivery.
Essential Duties & Responsibilities
· Provide outpatient primary care services to pediatric, adult, or all-age populations depending on training
· Conduct patient assessments including history taking, physical exams, diagnostic evaluations, and screenings
· Diagnose and manage acute and chronic illnesses within the scope of licensure and training
· Develop, implement, and evaluate patient-centered treatment plans
· Prescribe medications and order diagnostic testing in accordance with state and federal regulations
· Collaborate with supervising physicians, care teams, and specialists to coordinate care
· Document all clinical care in the electronic health record (EHR) with accuracy and timeliness
· Educate patients and families on diagnoses, treatment plans, preventive health, and self-care strategies
· Participate in population health initiatives and gap closure efforts
· Ensure compliance with SharpCare protocols, HIPAA, OSHA, and all applicable regulatory standards
· Maintain compliance with supervising/collaborating physician agreements and state scope of practice laws
· Participate in ongoing professional development and continuing education
· Attend required clinical meetings and contribute to interdisciplinary collaboration
· Mentor or precept medical students or new advanced practice providers as applicable
· Procedures included, but not limited to: PAP smears, IUD removals, joint injections, I & D, suture placement/removals, toenail removals
· Other duties as assigned
Qualifications | Competencies & Success Factors
· Required - Active and unrestricted license as NP or PA in the state of California.
· Required - Graduate of an accredited Nurse Practitioner or Physician Assistant program
· Required - 1+ years of clinical experience as a NP or PA in a related area.
· Preferred - Bilingual in Spanish or other commonly spoken languages.
