(Login for Information)a large multi-specialty staff model HMO, is seeking an Advanced Practice Provider to join our Edgewater facility.  (Login for Information)Monday – Friday 8-5, outpatient only, no weekends, call is light and by telephone only. The Advanced Practice Provider shall provide necessary and appropriate family medicine care, within the scope of the Nurse Practitioner's training and experience. Responsibilities and expectations to include the following:

• Deliver patient-centered, compassionate care
• Provide comprehensive medical assessments and treatments
• Develop personalized patient care plans
• Continuously enhance clinical knowledge
• Educate patients on disease prevention and health maintenance
• Collaborate with nurses, medical assistants, and other healthcare providers to ensure continuity of care
• Complete documentation, billing, and coding as required by healthcare regulations and standards
• Participate in quality improvement initiatives within the practice
• Attend staff meetings, training, and continuing education as required
• Participate in clinically driven initiatives to include annual complete physicals and quality and risk adjustment metrics

Qualifications

• APRN Florida licensure
• Minimum 3 years’ post licensing experience, working independently, in family medicine, urgent care clinics or a combination
• Graduate of an accredited program, APRN, NP, or FNP, program
