Family Practice Compensation Model

The following is a list of the compensation and benefits to be included in an employment contract.

"    Term    Twenty-Four (24) Months                
"    Base Compensation    $Competitive Base Salary (higher with experience)
"    Productivity Package    WRVU's > 4800 (MGMA Median) Paid at $33.00
"    Health Insurance    Covered by the Practice
"    Disability Insurance    $6,000 per Year for Documented Expenses
"    Dues/Membership/CME Fees    $5,000 per year for Documented Expenses
"    Malpractice Liability    Covered for Active Physicians; Tail Prorated Based on     Length of  Service
"    Personal Leave    4 weeks (20 days) Paid Time off for Sick/Vacation Time
   and 7 Holidays
"    CME Leave    2 weeks (10 days) Paid Time Off for Educational Purposes
"    Pension Plan    401(k) With a 4% Match capped at IRS Limit
"    Sign On Bonus    $15,000
"    Education Assistance    $20,000
