- Care delivered via telemedicine and home visits within a value-based care model
- Monday - Friday schedule, 8:30 am - 5 pm
- Full-time or part-time options available; minimum commitment of 3 days per week
- Serve as collaborating physician and first point of clinical escalation for Nurse Practitioners
- Focus on symptom management, goals-of-care discussions, and advance care planning
- Board-certified with experience in palliative care or leadership preferred
- Active Michigan license and DEA required
