- Covers 3 locations per day -- approx. 6 locations per week
- Total patient panel of 340 patients across 8 locations
- Multi-site facility coverage -- 8 locations throughout the week
- Patient population: ages 55 and older
- Chronic disease management required -- HTN, DM, CHF, COPD, CKD
- GI management required -- GERD, constipation, malnutrition, incontinence
- Geriatric post-acute care experience required
- Leave coverage -- 6-week assignment

From $70.00 to $95.00 hourly
