The Nurse Practitioner is a member of the provider team performing a variety of direct patient services including assessment, diagnosis, and implementation of treatment, evaluation and monitoring the general clinical/patient management plan for the medical clinic.

ESSENTIAL DUTIES & RESPONSIBILITIES:

• Provide direct patient care to a panel of patients accross the lifecycle.
• Assess, diagnose, prescribe, implement treatment, evaluate and monitor patients' health status as designated by the collaborative agreement established with a physician.
• Maintain patient confidentiality in all matters and provide patient with needed information.
• Works closely with Care Team to ensure preventive, chronic and acute care needs are being met.
• Participates in the Center's ongoing quality improvement initiatives including meeting clinical standards, metrics for the UDS report and health care plan goals.

EDUCATION, TRAINING AND EXPERIENCE:

• Must possess a MS degree from an acccredited nuring school with a Family Nurse Practitioner Track focus. Must have a certification or eligible for certification as an Advance Practice Nurse that is recognized by a national certifying body as approved by the IL Dept. of Regulation Advance Practice Board and have a current and valid Illinois license as an AdvancePractice Nurse with a DEA number and BLS certification.
