As a Neighborhood Physician at our Program of All Inclusive Care for the Elderly (PACE) you will be part of a Interdisciplinary Care Team that is focused on providing the very best in patient experience when it comes to receiving comprehensive medical care.  Amazing patient experiences begin with amazing providers.  Our patients know that every time they walk through our doors, they can expect quality treatment from professionals who care.  Through our values of compassion, integrity, professionalism, collaboration, and going above and beyond we ensure that all patients receive the very best care and support possible.  Making a difference in our community is made possible by the strong members that comprise the Care Team for each patient which include nurses, medical assistants, and scribes.

• As a Neighborhood Physician, you may be required at times to provide oversight and supervision to advanced practice providers, residents, rotational students, etc.
• The PACE Physician attends to the acute needs of the PACE participant, while managing the chronic medical problems, and paying attention to the preventive and rehabilitative care required by the PACE participant
• The PACE Physician incorporates the entire interdisciplinary team as appropriate to provide the medical care in a holistic manner, as is the hallmark of the delivery of care to the PACE participant
• Is responsible for prescribing appropriate medication, health regimens, and lifestyle adjustments that will contribute to their recovery from diagnosed health issues, prevent future health issues, and promote a healthier life
• Is comfortable managing stable patients with chronic conditions by utilizing internal resources, such as e-consult services, and other references to maintain their general health without the need/urge to refer patients to outside specialty care providers. Is able to triage and quickly recognize when treatment is appropriately provided on site versus in the urgent care or emergency room
• Responds to and provides crisis intervention services when needed; ensures behavioral health services are delivered in a culturally appropriate fashion, and is consistent with State of California laws and regulations, professional standards of care, agency guidelines, and professional ethical guidelines
• As a Neighborhood Medical Provider, participates in providing oversight and supervision to advanced practice providers, residents, rotational students, etc.

Qualifications:

• Licensed to practice Internal Medicine in the State of California
• In good standing with the Medical Board of California 
• Valid DEA Certificate
• Board certified
• At least 1 year of experience working with the frail and elderly/chronically ill population  
• Previous PACE experience and working with an interdisciplinary team preferred.

Pay range: 

• Internal Medicine- $281,430-$380,759, per year depending on experience. (DOE)
