Internal Medicine-Geriatrics Hospital Based job located in Pennsylvania

Facility seeking a Geriatric Medicine Physician for an outpatient position
This position will be housed primarily at the facility, however, occasional home care work may be required 
The schedule for this full-time opportunity is Monday – Friday, 8:00 am – 4:30 pm. 
On average, there are seven (7) on-call days (evenings/weekends) with weekend telephone triage.
Position serves as Outpatient PCP and skilled facility
Number of patients that the provider will be seeing in a day – varies from 2-8 per day
Provide initial and ongoing participant assessments, care plan development, and implementation of care plan.
Integrate the primary care treatment plan into the overall plan of care developed by the interdisciplinary team. 
Interacts with team members to meet emergent and acute need of participants. 
Evaluates and treats participants Primary Care Physician during acute illness.
Participate in provider on-call rotation as required

Requirements: minimum of one year working as a physician with the frail or elderly population, BC in Internal Medicine or Family Medicine, NJ license, CDS, DEA, BLS, clean/clear malpractice preferred
