Requirements: 
--Board Certification
--Nebraska License – Required or IMLC (LOQ in hand) 
--Malpractice – Clean Preferred 
--Availability at time of name clear – Required 
--Required Procedures: Nerve Blocks 

Assignment Details:
--Duties: General anesthesia cases, some OB cases, Neuro IR, No Pediatric cases, no cardiothoracic cases 
--EMR System: EPIC 
--Support Staff: Care Team Model with CRNA and Clinic Director assistance throughout the day 
--Beds: 187 
--Trauma Level / Academic Center: 1 
--Setting: 15 ORs, 3 endoscopy suites, 2 EP Lab / TEE / Invasive & Non-invasive Cardiology / Neuro IR / Bronch / MRI Lines / Obstetrics 
--Credentialling Timeframe: 60 – 90 days
