About the Role
We are seeking licensed US-based Physicians to deliver high-quality virtual care for patients with chronic conditions, primarily diabetes and cardiovascular diseases. This role focuses on ongoing patient management through teleconsultations, Chronic Care Management (CCM), and Remote Patient Monitoring (RPM), ensuring continuity of care and improved health outcomes.

Key Responsibilities
• Conduct telehealth consultations (video/audio) for patients with chronic conditions
• Manage and monitor patients enrolled in Chronic Care Management (CCM) programs
• Review and act on Remote Patient Monitoring (RPM) data (e.g., glucose, blood pressure)
• Develop and maintain individualized care plans
• Perform regular follow-ups and adjust treatment plans as needed
• Prescribe medications and ensure adherence to clinical guidelines
• Collaborate with care coordinators and support staff to optimize patient outcomes
• Maintain accurate and timely clinical documentation in the EMR Qualifications
• MD or DO degree from an accredited institution
• Active and unrestricted medical license in multiple US states (multi-state licensure preferred; ability to practice nationwide is a strong plus)
• Board-certified or board-eligible in Internal Medicine, Family Medicine, or relevant specialty
• Experience in managing chronic conditions, particularly diabetes and cardiovascular diseases
• Familiarity with CCM and/or RPM programs is preferred
• Prior telehealth experience is a plus
