Post-Acute Care Physician Locums Job in Southern California
Locations:
--Long Beach, CA
--San Diego, CA
Overview:
Our client is seeking experienced and compassionate providers to join their post-acute transitional care team in Southern California. These positions focus on caring for patients recently discharged from the hospital who require additional medical support and physical therapy before returning home. The care environment is similar to a skilled nursing facility (SNF) and emphasizes rehabilitation and recovery.
Position Details:
--Schedule: Monday–Friday
--Clinic Hours: 8:00 AM – 5:00 PM
--Call Hours: 7:00 AM – 7:00 PM (triage nurse available to screen calls and connect providers as needed)
--Average Census: 13 patients per day
--Sites: Two facilities per day, approximately 10 minutes apart
--Patient Population: Primarily elderly, with some middle-aged post-hospitalization patients
--Typical Length of Stay: ~2 weeks; some patients remain longer or through end-of-life care
--EMR: PointClickCare (primarily used for note documentation)
Support Structure:
--Dedicated triage nurse to handle incoming patient calls
--Collaborative care environment with on-site therapy and nursing support
Requirements:
--Active and unrestricted California license (MD, DO, NP, or PA as applicable)
--DEA and relevant certifications in good standing
--Experience in post-acute care, hospitalist medicine, or SNF settings preferred
