- See 10 - 12 patients daily with strong APP support
- Minimal call with nurse triage support system
- Value-based care experience preferred
- Board-certified or board-eligible in internal medicine with geriatric focus required
- Experience with HEDIS measures and HCC coding valued
- Spanish language skills appreciated but not required
