• Primary Care Provider for Boca and Hollywood

(Login for Information) is seeking a motivated and patient-focused Primary Care Physician (MD/DO) to join our growing outpatient primary care team in South Florida. This position is primarily based in our Boca Raton office, with openness to travel to our Hollywood location up to two days per week (approximately 35 minutes from Boca Raton).

The ideal candidate is experienced in outpatient primary care, comfortable managing a diverse patient population, knowledgeable in value-based care initiatives, and proficient in delivering high-quality, evidence-based medicine while maintaining strong patient relationships.

• 8:00 AM – 5:00 PM

Responsibilities

• Provide comprehensive outpatient primary care services for adult and geriatric patient populations
• Diagnose and treat acute and chronic medical conditions
• Perform preventative care visits, Annual Wellness Visits (AWVs), and routine physicals
• Manage chronic diseases including hypertension, diabetes, COPD, CHF, hyperlipidemia, and other complex conditions
• Ensure accurate and timely documentation within the electronic medical record system
• Utilize evidence-based medicine and preventative care guidelines
• Participate in value-based care initiatives including quality metrics, HCC/RAF coding accuracy, care gap closures, preventative screenings, and population health management
• Review laboratory, imaging, and specialist reports in a timely manner
• Coordinate care with specialists, hospitals, home health agencies, and ancillary providers
• Maintain compliance with all federal, state, payer, and company clinical standards
• Collaborate with clinical and administrative leadership to improve patient outcomes and operational efficiency
• Support patient satisfaction initiatives and provide compassionate, patient-centered care
• Participate in team meetings, workflow initiatives, and office collaboration efforts

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution
• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Active and unrestricted Florida medical license
• DEA license in good standing
• BLS certification required
• Primary care experience preferred
• Experience with outpatient/internal medicine/family medicine strongly preferred
• Experience with eClinicalWorks (eCW/ECW) preferred
• Knowledge and understanding of Value-Based Care (VBC), HCC coding, RAF scoring, quality measures, and preventative care initiatives preferred
• Strong clinical judgment and communication skills
• Ability to work collaboratively within a multidisciplinary team environment
• Bilingual (English/Spanish) is a plus but not required
