Job Description

(Login for Information), a community-based healthcare network serving southern Wisconsin and northern Illinois, is seeking a Family Medicine Nurse Practitioner to join our team at the (Login for Information) Clinic in Stockton, IL.

Opportunity:

  • 1.0 FTE (5-day work week) with flexible day-off scheduling to support work-life balance

  • Rural outpatient-only practice with a loyal, established patient panel

  • Minimal on-call responsibilities

  • Patient-centered scheduling averaging at least two patients per clinical hour, allowing time for relationship-based care

Responsibilities:

  • Provide comprehensive outpatient primary care services to patients across the lifespan

  • Conduct patient evaluations including detailed health histories and physical examinations

  • Diagnose and manage acute and chronic medical conditions

  • Develop and implement individualized treatment plans in collaboration with the supervising physician

  • Order, interpret, and follow up on diagnostic tests and lab results

  • Prescribe medications and therapies in accordance with state regulations and best practices

  • Promote preventive care and wellness, including screenings and immunizations

  • Educate patients and families on disease management and healthy lifestyle choices

  • Maintain accurate and timely documentation within the EMR

  • Collaborate with care teams and refer patients to specialists as needed

Qualifications:

  • Strong clinical skills with the ability to work both independently and collaboratively

  • Excellent communication and interpersonal skills

  • Commitment to compassionate, patient-centered, evidence-based care

  • Interest in practicing in a rural healthcare setting

Licensure/Certifications:

  • Graduate of an accredited Nurse Practitioner program

  • Current Illinois APRN license (or eligibility)

  • DEA license (or ability to obtain)

  • NP experience strongly preferred
