Responsibilities

• Function autonomously to care for patients with acute, critical, and chronic health conditions.
• Coordinate care management proactively, working with the healthcare team and facility partners to produce good outcomes.
• Provide medical oversight of patient care and rehabilitation.
• Ensure proper progress on plan of care and identify any early warning signs for readmissions during the critical, immediate post-discharge period
• Coordinate appointments for evaluation by specialists as needed for patients during their stay at the care facility
• Work individually and in interdisciplinary teams of resources and consultants
• Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers
• Record and maintain patient records in a timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams
• Timely documentation, discharge summaries, medicine reconciliation, etc., in EMR
• Talk to patients about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further care

• Ability to effectively collaborate and build relationships with patients, families and all care providers
• Proven organizational skills and ability to prioritize effectively.
• Strong computer skills with knowledge and experience with Electronic Medical Records

Qualifications

• Current unrestricted licence MD/DO
• Current and unrestricted DEA certificate
