POSITION SUMMARY: A Primary Care Physician (PCP) at Shalom fulfills a critical role in patient care, diagnosing and treating illnesses, prescribing medications, and overseeing treatment plans. They conduct physical examinations, interpret medical histories, and collaborate within health care teams to provide comprehensive and compassionate care. They will continually adapt treatment methods, prescribe tests, and engage in effective communication with patients and health care professionals to promote overall well-being and disease prevention. Their responsibilities include maintaining detailed records, staying up to date with medical research, and adhering to regulations, all aimed at ensuring high-quality patient care.

ESSENTIAL DUTIES:

  1. Working within our Patient-Centered Medical Home (PCMH) care model, provides effective and culturally sensitive integrated health care delivery at our primary care centers (PCC).
  2. Assesses, diagnoses, and administers treatment plans, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.
  3. Participates in after-hours call coverage according to the rotational schedule.
  4. Coordinates in-house integrated services with care teams comprised of nurses, behavioral and mental health professionals, social workers, community health workers, pharmacists, and other health care providers.
  5. Maintains understanding of all medical regulations and requirements.
  6. Serves as collaborating physician with mid-level practitioners within care team.
  7. Completes continuing education requirements, maintains professional affiliations, and enhances own professional growth and development.
  8. Provides management and administrative oversight of clinical care team activities in the absence of the Chief Medical Officer (CMO).

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Medical degree (M.D. or D.O.) from an accredited medical school
  • Family medicine board certification
  • Active, non-probationary, unrestricted license to practice medicine in the State of Indiana
  • Experience serving the medical and cultural needs of minority populations
  • Experience working with interdisciplinary teams
  • Admission privileges at area hospital
  • Previous experience in patient-centered medical home (PCMH) model preferred
  • Management of primary care practice a plus
