Family Medicine Hospital Based job located in Indiana

Facility seeking a BC/BE Family Medicine Physician to join established practice
--Join 4 physicians and 2 advanced practice providers who enjoy a supportive work environment and a great work-life balance
--Schedule: Monday - Friday, 8:00 a.m. – 5:00 p.m.
--Weekday call is rotated among the physicians: 1:7
--Weekend Call: 1:14
--Average outpatient volume: 20 - 25 patients per day
--No inpatient work or OB
--Common Diagnoses:  Diabetes,  Hypertension,  COPD
--Wellness Visits
--On-site Equipment: Lab and Imaging
--Sports Medicine located in same building
--Outpatient physical therapy
