Job Description

Position Summary

We are seeking a compassionate and mission-driven Internal Medicine or Family Medicine Physician to join our multidisciplinary care team at a Federally Qualified Health Center (FQHC). The physician will provide comprehensive, patient-centered primary care services to a diverse and underserved population, regardless of patients’ ability to pay. This role supports our commitment to high-quality, coordinated care within a Patient-Centered Medical Home (PCMH) model.

Essential Duties & Responsibilities

  • Provide comprehensive primary care services, including preventive care, chronic disease management, and acute care for adult or family populations
  • Diagnose and treat a wide range of medical conditions in an outpatient setting
  • Collaborate with behavioral health, care management, nursing, and outreach teams to ensure coordinated, whole-person care
  • Participate in quality improvement initiatives, population health management, and performance measures
  • Maintain accurate, timely documentation in the electronic medical record (EMR)
  • Provide patient education and counseling to promote wellness and self-management
  • Comply with all FQHC, state, and federal regulations, including HRSA requirements
  • Participate in on-call coverage as assigned (if applicable)

Skills & Qualifications

  • MD or DO from an accredited medical school
  • Board Certified or Board Eligible in Internal Medicine or Family Medicine
  • Active (or eligible) state medical license
  • DEA registration (or eligibility to obtain)
  • Experience in primary care, community health, or underserved populations preferred
  • Strong commitment to patient-centered, culturally competent care
  • Ability to work effectively in a team-based, fast-paced environment
