Family Medicine Hospital Based job located in Michigan

Seeking a full-time Family Medicine or Internal Medicine Physician to join established outpatient clinic practice 
--Monday – Friday | 40-hour weekly schedule with 36 patient-contact hours and 4 administrative hours
--Provide full-spectrum outpatient primary care services to adult and/or family medicine patients 
--Diagnose, treat, and manage acute and chronic medical conditions 
--Conduct preventative care visits, annual wellness exams, and routine follow-up appointments 
--Develop long-term treatment plans and coordinate patient care with specialists when appropriate 
--Maintain accurate and timely documentation utilizing EPIC EMR system 
--Collaborate with multidisciplinary care teams to ensure high-quality patient outcomes 
--Participate in shared on-call responsibilities as assigned 
--Maintain productivity expectations during 36 weekly patient-contact hours 
--Utilize 4 administrative hours weekly for charting, care coordination, and related duties 
--Deliver evidence-based, patient-centered care in a supportive outpatient clinic environment 
--Participate in quality improvement initiatives and organizational standards of care 
--Build strong relationships with patients and the local community 
--Support continuity of care and patient education initiatives 
--Adhere to all organizational compliance, credentialing, and clinical standards
--Annual salary range: $235,000 – $265,000
