This community-based organization provides innovative, comprehensive health care and social services of the highest quality. This is a full-time salaried position, 4-5 days a week with no after-hours call. The practice uses Centricity EHR. Providers typically see 15-20 patients a day, conduct evening office hours once a month and weekend clinics once or twice per year. No inpatient care. On average 1-2 patient home visits are conducted per month. The practice also cares for patients at two area nursing homes. The practice is committed to fostering long-term physician/patient relationships. Practitioners are encouraged to participate in the education of health care students, and to remain life-long learners.
This Patient Centered Medical Home in northern NH was founded in 1974 and currently employs a well-rounded staff of physicians, NPs and PAs in specialties including Family Practice, Pediatrics, Internal Medicine and Podiatry.
