We are looking to hire Family Practice Physicians who provide comprehensive primary and preventative health care and disease management to individuals within a discipline of certification (Adults, Pediatrics, Family, and Women’s Health). The incumbents in this job have routine access to individually identifiable health information including the patients’ entire medical records on a need-to-know basis only.
Qualifications/Education/Experience
- Performs primary and preventative health services and disease management to individuals as a general practitioner following the chronic care model.
• Prepares and reviews case histories and obtains data through patient interviews.
• Participates in the provider on-call schedule.
• Participates in in-patient rounds as necessary.
• Performs office-based procedures.
• Educates patients on procedures performed and the importance of follow-up.
• Maintains accurate and legible medical records in compliance with NCHC policies.
