Family Medicine-Primary Care Physician - BE/BC

Location: Waterloo, NY (Finger Lakes Region) Setting: 160-Bed Skilled Nursing Facility

Introduction:

(Login for Information) seeks a dedicated Board Eligible/Board Certified Family Medicine Physician to provide primary care services at our partner, a 160-bed skilled nursing facility (SNF) located in Waterloo, NY, within the scenic Finger Lakes region. This is an excellent opportunity for a physician interested in focusing on the care of geriatric patients within a long-term care setting.

Role Summary:

As the Primary Care Physician for residents at this skilled nursing facility, you will be responsible for providing comprehensive medical care, managing chronic conditions, coordinating with specialists, and collaborating with the facility's interdisciplinary team (nursing, rehabilitation, social services) to ensure high-quality, resident-centered care.

Key Responsibilities:

• Provide ongoing primary medical care to residents within the skilled nursing facility.
• Conduct comprehensive geriatric assessments, including history taking and physical examinations.
• Diagnose and manage acute and chronic medical conditions common in the elderly population.
• Develop, implement, and regularly review individualized care plans for residents.
• Prescribe appropriate medications and treatments, monitoring for effectiveness and side effects.
• Collaborate with nursing staff, therapists (PT/OT/SLP), social workers, dietitians, and consulting specialists to provide coordinated care.
• Participate in care planning meetings and communicate effectively with residents and their families.
• Maintain accurate and timely documentation in the facility's medical record system.
• Adhere to all facility policies and relevant state/federal regulations for long-term care.

Qualifications:

• Education: Medical Degree (MD or DO) from an accredited institution (Required).
• Training: Completion of an accredited Family Medicine residency program (Required).
• Certification: Board Eligible or Board Certified (BE/BC) in Family Medicine (Required). (Geriatric certification or experience a plus) .
• License: Eligible for or possess an active, unrestricted New York State medical license and DEA registration (Required).
• Experience: Experience or demonstrated interest in caring for patients within a Skilled Nursing Facility (SNF) or geriatric setting (Strongly Preferred).
