About the Opportunity

Our client is seeking a Family Medicine Physician (MD/DO) to join a growing primary care practice serving geriatric patients in senior living communities throughout Delaware County, PA. This is a part-time opportunity (20–24 hours per week) with the potential to grow into a full-time position as the practice continues to expand.

The physician will provide care across multiple senior living facilities throughout Delaware County, including the Havertown and Upper Darby areas. The ideal candidate will have experience in primary care, a passion for caring for older adults, and a collaborative, patient-centered approach.

Minimum Requirements

  • Current, clear, and active Pennsylvania medical license (MD or DO)
  • Maintain compliance with all applicable licensing, certification, and accrediting bodies
  • Experience in primary care preferred; geriatric experience is a plus
  • Excellent organizational skills and ability to collaborate with clinical team members
  • Familiarity with telehealth platforms, billing systems, coding standards, and care coordination programs is preferred
  • Strong communication skills with geriatric and disabled patient populations
  • Eligible to participate in Medicare and Medicaid programs (not sanctioned)

Responsibilities

  1. Provide compassionate primary care services for geriatric and/or disabled patients, including comprehensive and focused physical examinations, health assessments, diagnosis, and treatment of acute and chronic illnesses.
  2. Manage chronic conditions such as diabetes, hypertension, congestive heart failure, arthritis, chronic obstructive pulmonary disease, and thyroid disorders.
  3. Order, interpret, and evaluate laboratory and diagnostic testing as appropriate.
  4. Collaborate with specialists and other healthcare providers through appropriate referrals.
  5. Assist in coordinating community resources and follow-up care for patients and families.
  6. Educate patients on health promotion and wellness throughout the aging process.
  7. Establish and monitor appropriate levels of care for patients.
  8. Complete all medical record documentation prior to the end of each shift.
  9. Partner with the Medical Director to ensure operations remain consistent with professional and medical standards of care.
  10. Identify opportunities for growth and enhancement of existing or new services.
  11. Meet with the Medical Director or designee to review patient care quality, policies, procedures, outcomes, and records.
  12. Participate in quality management initiatives and support clinical staff proficiency in patient care activities.
  13. Conduct patient follow-up calls as needed to ensure continuity of care and patient well-being.
  14. In the absence of the Medical Director, provide guidance and support to medical personnel and clinical colleagues.
