COMPENSATION AND BENEFITS Family Medicine (FM)

• $275,000 Base Salary
• $30,000 Sign on Bonus
• $5,000 relocation assistance
• Student Loan Repayment

• CMES - 5 days with $4000 allotted

RESPONSIBILITIES AND FACILITY DETAILS Family Medicine (FM)

• 100% Outpatient
• 4-day work week
• FQHC - HPSA score of 22
• 15-17 patients daily
• Call 2x monthly - Phone Consults only

QUALIFICATIONS AND SKILLS Family Medicine (FM)

• Open to New grads and or experienced
• BE/BC
• MD/DO
• Must acquire WI license
• Able to sponsor visa Candidates
