A respected PACE program is expanding with a new clinic in El Sobrante and seeking a Primary Care Physician (MD) to join the team. What You’ll Actually Do:

• Be the trusted physician for a panel of older adults in our PACE program.
• Lead and collaborate with a talented interdisciplinary team (NPs, PAs, nurses, social workers, therapists).
• Take your time, conduct thoughtful assessments, design care plans and see the difference they make in patients’ lives
• Guide specialist referrals, coordinate hospital care, and oversee smooth transitions back home.
• Use state-of-the-art technology that makes your job easier, not harder.
• Contribute to a culture of kindness, respect, and teamwork.

Who You Are (a.k.a. Our Dream Match):

• Must hold an MD degree with a current California license with DEA and BLS certification.
• Fellowship training in geriatrics or experience working with older adult or frail populations preferred (new grads with relevant training will be considered).
• Passionate about caring for older adults and excited to work in a PACE model.
