Job Description
Seeking a BC/BE Palliative Care Physician to join the Advanced Heart Failure Center
--The Advanced Heart Failure-Specialized Palliative Care Physician will be working alongside the cardiac team of AHF cardiologists and cardiac surgeons and joining the Palliative Care team that consists of 4 physicians and 13 APRNs.
--Candidates for this position will be interested in doing coordinated care, medication & chronic symptom management, end of life management, as well as goals of care discussions.
--The primary patient population for this role will be heart failure; this includes hospitalized patients in cardiogenic shock supported with ECMO, Impella and left-ventricular assist devices, as well as consultations of ambulatory patients in Advanced Heart Failure Center
--There will be some responsibility for general medical/surgical and oncology patients as needed to support the overall team and community.
--This is a newly established Advanced Heart Failure program and a great opportunity for someone with an interest in development, program growth and out of the box thinking with a high focus on exceptional patient care to join the team and assist with this strategic redesign process.
