Summary
The Nurse Practitioner (sometimes referred to as an “Advanced Practice Nurse”) provides an enriched level of medical care with a goal of increasing wellness, prevention of illness and improving outcomes of care through early detection, wellness visits, and more timely diagnosis and/or treatment; Works with a multi-disciplinary team and individualizes treatment plans as residents move from practitioner(s) to provider(s) outside an (Login for Information) community, and upon returning home. In addition to resident care, the Nurse Practitioner provides bedside education for staff. The Nurse Practitioner works in collaboration with the community Medical Director in providing care and services to residents; Practices within the scope of licensure and in accordance with the state Nurse Practice Act and state and federal licensure rules and regulations for the level of care in which they practice; Practices in accordance with (Login for Information) clinical and administrative policies and procedures and in accordance with the Collaborative Practice Agreement.
Responsibilities
1. Performs physical examinations and institutes preventive health measures
2. Orders, interprets, and evaluates diagnostic tests to identify and assess resident’s clinical problems and health care needs
3. Records physical findings, and formulates plan and prognosis, based on patient's condition utilizing electronic health records. Maintains medical records and submits at least 10% of records to the Medical Director for bi-weekly review in accordance with the “Collaborative Practice Agreement”
4. Records all services performed for accurate billing for services provided in compliance with all local, state and federal laws, as well as payor rules and regulations; Submits billing documents to billing agency in a timely fashion
5. Works with the Medical Director, other in-house physicians, staff, residents and family members to develop a comprehensive resident care plan and reviews plan periodically with resident’s physician
6. Prescribes, recommends, or discontinues drugs and treatments in accordance with the “Collaborative Practice Agreement” and the state’s “Nurse Practice Act”; Assists (Login for Information) in establishing formularies for medication and other therapeutic measures
7. Prescribes therapy such as physical therapy, respiratory therapy, or related therapeutic procedures
8. Refers patients to Physician for consultation or to specialized health resources for treatment with a goal of achieving better outcomes in multi-disciplinary treatment plans; Certifies and re-certifies resident for Medicare stays
9. Looks for ways to improve and promote quality; Provides bedside teaching and/or clinical education programs to clinical staff if requested; Collaborates and advises with (Login for Information) Home Office clinical team in establishing best practices; Collects and analyzes data to identify best practices, state averages, and Medicare guidelines
10. Maintains credentials and licensure with all major insurance companies and licensing boards with the assistance of (Login for Information) and billing agency
11. Follows all procedures and protocol as per (Login for Information) policy, federal and state regulations, standard practice and in accordance with the “Collaborative Practice Agreement”
12. Responds to emergencies in licensed areas, or in Independent Living, when available
Qualifications, Knowledge, Skills & Abilities
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Current Missouri RN licensure and NP License
- Certification as a Geriatric and/or Adult Nurse Practitioner
- Palliative Care certification required if working in the palliative care program (May be obtained after employment)
- Successful completion a Nurse Practitioner program
- Must be CPR BLS (basic life support) certified with the American Red Cross
- Prior experience in Geriatrics preferred
- Ability to read, analyze and interpret general and professional documents; Ability to write routine reports and correspondence; Ability to effectively present information in one on one and small group situations; Must have proficient computer skills; Must possess the ability to make independent decisions when circumstances warrant such action
