Job Description

Place of Performance: Rocky Mountain Regional VA Medical Center (RMRVAMC), 1700 N. Wheeling Street, Aurora, CO 80045

Hours of Performance: Monday-Friday, between 7:30am and 5:30pm (Exact shifts may vary by team assisgnment, but will remain within standard daytime hours.) No evenings, weekends or holidays unless otherwise requested. 

Minimum Requirements: Education: Graduation from a school of professional nursing approved by the appropriate state accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission of Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). License: Current active license in at lease one (1) state, territory or commonwealth in the U.S. or District of Columbia. Experience: Minimum 2 years of direct patient care experience withing the last 3 years. Certification: Basic Life Support (BLS)

Core Duties: Provide direct and indirect nursing care, by contributing to the patient's assessment, determinig priorities, identifying nursing measures and therapeutic objectives and evaluating outcomes. The position requires independent judgment and nursing actions, flexibility, organizational skills and the abiltiy to utilize an interdisiplinary approach in the provision of nursing care. Review consults or requests for completeness and appropriatness/utilization review. Dispositioning care in the community per utilization review and guidelines. Clinical review of incoming medical records documenting communication with ordering providers and PCPs. Coordination of care, corresponding with veteran, community providers, internal providers/nurses, and admin staff to ensure the care and authorization for care requested is processed. Triage specialty consults of clinical appropriateness and completeness within the National RCI Standards for Coordinated Care. Communicate with vererans on their health care options via phone calls to determine receiving care at the VA facilty or through the community eligible providers so the veterans can make informed decisions on how/when/where to receive specialty care if they are eligible for CC. Provide clinical knowledge to the veterans and act as a clinical resource to multidisciplinary staff (MDs, LIPs, SW, HAS ect). within our VISN19 and outside the VISN. Collaborate with CACs to rebuild/modify consult templates. Serve as a liaison to both internal staff and CC staff during the consult process. Serves as a liaison to the ordering provider and specialty care services that RCI is implemented into. Serves as delegation of authority by building trusting relationships with the specialty providers in the clinic into based on vetted RCT Specialty Triage Tools with the VA specialty providers we collaborate. Attend staff meetings as required by the VAMC Chief of Service, Chief of staff, or designee. Follow all established patient safety and infection control standards of care.
