Must be BC/BE in Internal Medicine

Schedule: 2 weeks on, 1 week off (It’s not 12 hour shifts! They typically work 10 hours, but varies depending on the patient census.) Dayshift only and one weekend. 

Call Schedule: With the additional hospitalists, the minimal night call from home will be 1:6.

This is NOT an acute setting. This Hospitalist position is for when patients are discharged from the hospital but aren’t ready to go home just yet. There are no procedures involved - if anything major happens to the patient, they stabilize them & send them back to the hospital right away. 

This Post-Acute Hospitalist opportunity offers a unique practice which combines managing a wide range of medical and surgery sub-acute patients during their transition to home. Our facility is the only hospital in the region to be accredited by both the Commission on Accreditation of Rehabilitation Facilities (CARF) and The Joint Commission. 

Responsibilities include coordinating all aspects of care for the TSU (Transitional Sub-Acute Unit) patients and provide co-management services for TRC (Transitional Rehabilitation Center) patients:

Admit and manage the clinical care of Adult Medicine Service patients on a sub-acute unit.
Discussion of End of Life preferences.
Comprehensive medical consults with sub-specialists on acute rehab patients.
Coordination of care with primary care physician.
Discharge planning, including ensuring appropriate follow-up is arranged.
Coordination/co-management of care with physiatrist on all of their patients.
Participate in quality improvement activities.
Manage patients in a high-quality, cost effective fashion.
Inpatient attending for Internal Medicine Residents.

Salary Info:

Base - $185,000 + higher with experience
Quality Incentive – Up to $20,000
Productivity (based on WRVU)
