The APC is responsible for evaluation, diagnosis, treatment, and ongoing management of acute and chronic medical conditions in the inpatient setting, ensuring safe, efficient, and evidence-based care.

About the Role
- Perform comprehensive history and physical examinations for admitted patients
- Manage acute and chronic medical conditions in hospitalized adults
- Develop, implement, and adjust treatment plans in collaboration with supervising physicians
- Order, interpret, and act on diagnostic tests (labs, imaging, EKGs, etc.)
- Perform procedures as credentialed
- Respond to clinical deterioration, rapid responses, and medical emergencies as required
- Participate in admissions, daily rounding, transfers, and discharges
- Conduct medication reconciliation and ensure safe prescribing practices
- Collaborate with physicians, nursing staff, case management, pharmacy, therapy services, and consultants
- Communicate effectively with patients and families regarding diagnoses, treatment plans, and goals of care
- Participate in multidisciplinary rounds and discharge planning
- Provide thorough handoffs during transitions of care
