Primary Care Physician

As a Primary Care Physician, you will conduct assessments, develop care plans, and provide ongoing medical care to seniors within a collaborative environment.

Responsibilities:

  • Comprehensive Assessments: Conduct in-person assessments for new participants, semi-annual evaluations, and unscheduled visits to ensure ongoing health and well-being.
  • Collaborative Care Planning: Partner with the IDT to develop and implement personalized care plans that address each participant's unique needs and goals.
  • Discharge Planning: Collaborate with community-based physicians and the IDT to facilitate smooth transitions for participants returning home from acute or long-term care settings.
  • Ongoing Care: Help maintain the care of participants in nursing homes, making regular visits as needed.
  • Care Coordination: Work closely with physicians, community-based PACE physicians, and the IDT to oversee and ensure effective care coordination across all settings.
  • On-Call Coverage: Participate in a rotating night and weekend call schedule to provide continuous medical coverage to participants.

Qualifications:

  • Required:
    • Master's Degree/Doctorate Degree.
    • Board Certification – Internal Medicine or Family Medicine.
    • Active California Physician License without Restrictions.
    • Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education (ACGME).
  • Preferred:
    • Board Certification – Geriatrics.
