Primary Care Physician
Location: La Quinta, CA

(Login for Information) is seeking a dedicated Primary Care Physician to join a dynamic healthcare team at a medical facility in La Quinta, CA. This is a fantastic opportunity to provide comprehensive care to a diverse patient population while contributing to a collaborative and innovative team environment.

Responsibilities:

Patient Assessments: Perform in-person comprehensive assessments and provide continuous medical care.
Care Planning: Work with the interdisciplinary team (IDT) to create individualized care plans for each participant.
Discharge Planning: Collaborate with community-based physicians to facilitate smooth transitions for patients discharged from acute or long-term care.
Nursing Home Care: Oversee care for participants in nursing homes, conducting regular visits as needed.
Care Coordination: Ensure effective care coordination with physicians and providers.
Leadership: Lead and support Nurse Practitioners and IDT members.
On-Call Coverage: Participate in a rotating on-call schedule, covering nights and weekends.

Qualifications:

MD or DO from an accredited medical school
Completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
Board certification in Internal Medicine, Family Medicine, or Geriatrics (preferred)
Active California Physician License without restrictions
CDS Certification and DEA Registration
Eligible to provide services to Medicare and Medi-Cal participants
