Post Acute Care Physician opportunity in Middletown, DE

Great opportunity for a driven physician to join our post-acute care team in the Middletown, Delaware, area. This is a full-time opportunity (5 days/week) rounding in skilled nursing facilities, with excellent support and training, autonomy, and earning potential.

Key Responsibilities

• Perform primary care in a post-acute setting for short term and long-term patients
• Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
• Order and interpret lab results
• Prescribe and monitor medications
• Consult with physicians and coordinate patient care with other healthcare practitioners
• Educate patients and their caregivers on coping skills to handle chronic and age-related conditions

Requirements

• Current physician license (state of Delaware) and DEA
• Experience in post-acute, acute, ED, or clinic settings preferred but not required; new grads welcome to apply
• Strong clinical foundation and knowledge of healthcare regulatory standards
• Excellent communication and interpersonal skills
• Collaborative, team-oriented approach with a positive outlook
