- MAXIMUM 30-45 MIN COMMUTE IN CHICAGOLAND
We are seeking a dedicated and compassionate primary care, internal medicine, or family practice physician, MD or DO to provide on-site primary care services to our patients that reside in assisted living, memory care, and independent living senior communities. The position is managing stable chronic medical condition in senior citizen patients.
The ideal candidate will be experienced in primary care, willing to work in senior living community buildings located around 5 to 45 minutes from where they reside. It is helpful to have strong self-management skills, maintain strong relationships with building staff, and be able to see around 20 patients per day.
The position is an Independent Contractor job with our help setting up your own small business. Candidates are compensated in a pay-by-production model or salary-based structure.
Responsibilities:
1. Provide comprehensive primary care services to a senior citizen patient population across various acuity levels, including assisted living, memory care, independent living.
2. Conduct monthly health assessments and formulate treatment plans for acute issues and management of chronic diseases.
3. Collaborate with a multidisciplinary team of healthcare professionals to provide optimal patient care, including building nursing team, Executive Director, and Sales team.
4. Prescribe medication, order therapy or home health based on medical need to treat or prevent illness, disease, or injury.
5. Monitor and manage patient care plans, adjusting as necessary.
6. communicate with patients and their families on health maintenance, disease prevention, and the management of acute and chronic illnesses on a monthly basis.
7. Document patient care in our EMR based on compliance with regulatory billing standards and organizational policies.
8. Participate in quality improvement initiatives and contribute to the development and implementation of new health programs and services.
9. Maintain and provide a monthly updated building Patient Census along with maintaining a weekly patient list
Qualifications:
1. MD or DO with a unrestricted license in state of practice.
2. Valid Nurse Practitioner license and Registered Nurse license.
3. Excellent communication skills with building staff, patients, and their families.
4. Maintain strong relationships with building staff and department heads
5. Strong clinical judgment, diagnostic skills, and excellent patient management abilities.
6. Proficient in EMR. We provide training for ours called Tebra
7. Need excellent self-organizational skills keeping track of building patient census lists.
8. Ability to work independently and as part of a multidisciplinary team.
Compensation:
Compensation will be based on a flexible structure, with options for pay-by-production or a competitive salary.
Fulltime starting 200k up to 300k plus bonus opportunities with all holidays off and expected 1 evening 5pm-Midnight on-call per month.
