Job Description

JOB PURPOSE:

Under the direction of the Kinship Health Medical Director, the Regional Medical Director provides leadership and is responsible for all Kinship PACE medical services at the assigned regional and local level, including planning, development, implementation, and ongoing delivery of participant (PTP) care and clinical outcomes for the assigned region. Acts as a Primary Care Provider at assigned PACE Center.

JOB RESPONSIBILITIES:

  • Advise and inform Kinship Health Medical Director on Kinship PACE medical care development and operations at the regional and local level.
  • Works with the assigned Quality Specialist on local quality assessment and performance improvement program (QAPI) and reports to Kinship Health Medical Director on all clinical matters.
  • Attends local and regional QAPI meetings
  • Serve on Kinship PACE’s regional management team
  • Work with the regional program director in negotiating contract agreements with medical specialty and long-term health care providers following policies and procedures and contact practices.
  • Establish an array of medical service delivery contacts to complete the service network and set up the quality assessment and performance improvement infrastructure. Maintain positive relationships with network providers
  • Work with regional program director to recruit, interview, credential, hire, and orient primary care providers who can work cohesively as a member of the interdisciplinary team. Provide supervision and performance evaluation to maintain high-quality participant primary care
  • Provide consultation to the clinic nurse manager, home care coordinator, and other clinical staff, holding regular meetings to discuss participant care and clinic operations
  • Utilize interpersonal skills to maintain supportive relationships with all staff.
  • Participate in the continuing education and training of all regional Kinship PACE staff
  • Confer with regional program director on standards for interdisciplinary team care, participant and caregiver and satisfaction, quality, protocols, grievances and appeals, enrollment and disenrollment, infection and exposure control, emergency preparedness, data and medical records management, service access, and overall program performance
  • Assists in the development of Kinship Health policy and procedures on medical care, in collaboration with corporate staff.
  • Works with the regional program director to develop plans and committee oversight for medical care, utilization review, and ethics so that all Kinship PACE quality, medical, and program standards are met.
  • Act as Chair of the Quality Subcommittee to oversee and implement quality initiatives to ensure quality of clinical care, patient safety, and participant satisfaction.
  • Serve as a standing member of Kinship PACE’s local and regional Infection Control and Pharmacy and Therapeutics Subcommittees.
  • Recommends standards for medical care to the Medical Advisory Committee.
  • Communicates to the providers, staff, and contractors the standards of care and how they are to be implemented
  • Provide direct primary care, including inpatient management (as appropriate), routine assessments, reassessments and episodic care, and medical record documentation
  • Serve as a liaison between Kinship PACE and community providers, providing information and conducting presentations as required
  • Coordinate educational rotations for residents and fellow employees.
  • Responsible for the safety and health of Kinship PACE participants and employees. Promote safe work practices; conduct environmental rounds; attend to, investigate, and report accidents promptly; assist with medical surveillance; and provide Kinship Health-specific safety training. Intervene immediately when conditions exist that poses an immediate threat to participants, physicians, staff, contractors, visitors, and others
  • Understand fiscal issues and is appropriately involved in meeting budget targets
  • Consistently meet or exceed Kinship Health targets for productivity
  • Continually seek better ways for delivering services and communicating with participants
  • Consistently meet or exceed Kinship Health customer service targets.
  • Demonstrate respect for and promote participant rights including dignity, self- determination, access to care, confidentiality, and independence.
  • Understand the importance of community involvement and participate as appropriate in activities that link Kinship PACE to its communities
  • Effectively collaborate with staff peers and contractors to meet Kinship Health goals and to further success
  • Demonstrate proficiency in delivering age-specific care
  • Consistently meet or exceed Kinship Health quality assessment and performance improvement targets
  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.
  • Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance

QUALIFICATIONS:

  • Six Minimum of five (5) years of experience in caring for seniors
  • Minimum of two (2) years of documented experience working with a frail or elderly population in a long-term care setting
  • Graduate of an accredited school of medicine
  • Board certified in internal medicine or family practice. Board certification granted by the American Boards of Internal Medicine and Family Practice for Added Qualification in Geriatric Medicine preferred
  • Licensed by the State Medicine Board to practice medicine
  • Medicine Directors Certification preferred
