Post-Acute Care Physician (Part Time) opportunity in West Chester, PA

Great part-time (2 days per week) primary care role managing complex patient cases in a post-acute setting (SNF) in West Chester, Pennsylvania, focusing on improving care for some of the most vulnerable patients.

Qualifications:

• Minimum of 1 year of experience, with a commitment to delivering high-quality, patient-centered care
• Post-acute care experience is beneficial, though not required, as we provide comprehensive support and development opportunities
• Active and unrestricted Pennsylvania license (or in process)
• Active and unrestricted DEA/prescriptive authority (or in process)
• A deep commitment to improving patient outcomes, leading clinical excellence, and driving change in the healthcare landscape
• Internal medicine (IM), family medicine (FM), emergency medicine (EM), hospitalist or candidates with any related experience are welcome to apply
