Job Description
Seeking a BC/BE Geriatrician for a Traditional or Outpatient only position
• Monday – Friday 8am to 5pm but flexible on hours
• Clinic 4 ½ days allowing item for admin duties.
• This role requires a deep understanding of age-related conditions, complex medical needs, and multidisciplinary care to manage chronic conditions, prevent disease, and improve quality of life.
• The ideal candidate will have an interest in both clinical excellence and medical education, particularly in training and mentoring Internal Medicine residents.
• Conduct comprehensive evaluations of elderly patients, including medical history, physical examination, cognitive and functional assessments.
• Diagnose and treat age-related conditions, including chronic diseases like hypertension, diabetes, osteoarthritis, and heart disease.
• Prescribe appropriate medications, keeping in mind polypharmacy concerns and potential drug interactions.
• Develop personalized care plans to manage chronic conditions and prevent hospitalization.
• Work closely with a team of healthcare providers, including nurses, social workers, physical therapists, and dietitians, to coordinate patient care.
• Communicate with family members or caregivers to ensure understanding of the patient's condition and treatment plan.
• Educate patients and their families about aging-related health issues, self-management techniques, and preventive care strategies.
• Promote healthy lifestyle choices, including diet, exercise, and social engagement, to maintain independence and improve quality of life.
• Perform screenings for common health issues such as depression, vision or hearing impairment, osteoporosis, and cancer.
• Focus on preventive health measures such as immunizations, falls prevention, and advanced care planning (e.g., advance directives, end-of-life discussions).
• Monitor patient progress through regular follow-up visits, adjust treatment plans as needed, and address any emerging health concerns.
• Manage transitions of care between different healthcare settings (e.g., from hospital to home) to ensure continuity
• Serve as a preceptor for Internal Medicine residents in outpatient geriatrics settings.
• Participate in the development and delivery of GME-related activities including didactics, bedside teaching, and curriculum planning in geriatric medicine.
