Seeking BE/BC Family Medicine or Internal Medicine Physician with a passion to care for the elderly/senior population, or Geriatrician to provide primary care services to SR Care clients at participating LTC facilities and serve as the individual SNF's Medical Director. Work/Life fit: Enjoy a flexible, full time 4 or 5 day workweek with admin time, no after-hours call, no weekends Work Environment: Integration and management of care to include inpatient admitting, clinical, and admin services. SNFs Leadership: Shine as the Medical Director to our contracted SNFs. Advocacy: For patients and providers. Administration round tables with providers for input with strategic planning Teaching-Friendly Culture: Share your passion with residents and medical students Expected Volume: Seeing 14 patients assigned per day, typically at 1 facility per day.
