Job Description

This organization operates under the PACE (Program of All-Inclusive Care for the Elderly) model, where you'd work alongside an experienced interdisciplinary group that truly prioritizes patient outcomes over volume.

  • Join a mission-driven PACE organization dedicated to helping their members with complex needs remain independent in their communities
  • Practice within a fully integrated interdisciplinary model, collaborating daily with nursing, social work, therapy, and behavioral health to deliver all-encompassing care
  • Outpatient-focused role with a predictable Monday-Friday schedule and low-volume after-hours phone call rotation (6-9 weeks annually)
  • See between 2 and 9 patients per day on average allowing for extensive 1:1 time
  • Serve as a care team leader helping navigate complex cases and shaping individualized care plans
  • Fully supported by experienced clinical staff and advanced practice providers, helping create a collaborative and efficient care environment
