Family Practice Physician Soldotna, AK

 

Sign on bonus of $10,000

 

We are looking for a Board-Certified Family Practice physician to join non-profit medical group full-time in Soldotna, AK.  We need a physician who is willing to see patients from birth to death. 

 

This position exists to provide family practice medical care to our patient population in the Central Kenai Peninsula.  The physician would act as a member of our Healthcare Team in the provision of health services to individuals, families, and/or their communities. The physician will work collaboratively with patients and their caregivers—to the extent preferred by each patient—as well as with other members of the Healthcare Team to accomplish shared goals within and across settings to achieve coordinated, high-quality care.  The physician would provide ongoing guidance, support, and education to other members of the Healthcare Team as it pertains to ensuring effective, quality primary care for our patients and their families.

We are a non-profit, family practice Federally Qualified Health Center, located in Soldotna and Kenai Alaska with approximately 100 employees.  We offer medical, dental, and behavioral health services and serve a community that involves seven small towns with a total population of approximately 39,000 of which 25% are at or below 200% of the Federal Poverty Level. We serve a triangular geographic area of approximately 2,052 square miles that runs from the town of Ninilchik to Nikiski to Cooper Landing.  Our Mission is “Strengthening our Community by Providing Accessible and Affordable Healthcare.”  We have been providing high quality care for the community since 1976.

 

Our main office is in Soldotna, AK, where the beautiful Kenai River flows through the middle of town, drawing thousands of visitors each summer, to catch world-class salmon.  If you like the outdoors, we have plenty of wildlife here (eagles, moose, caribou, brown and black bears, dozens of different birds), close proximity to many different hiking trails, from easy to difficult, many which can be accessed year-round, with proper gear, world-class cross-country skiing right here in town and much more. 

 

 

ESSENTIAL FUNCTIONS/ROLES & RESPONSIBILITIES OF THE POSITION:

 

  • Evaluate and treat patients within the privilege range granted by the Board, and within the scope of training, capabilities, and license criteria as a health care provider
  • Provider may travel to homes of patients in special circumstances to deliver medical care as needed on an infrequent basis.  This may also include assisted living centers which are now considered the home of the patient. 
  • Provider shall apply for and obtain, if deemed qualified in accordance with existing standard procedures, appointment to the Medical Staff pursuant to the Bylaws of the Hospital's Medical Staff with clinical privileges appropriate to the family medicine services to be performed for periodic on-call Pediatric coverage
  • Be a patient advocate
  • Coordinate health promotional activities
  • Provide patient follow-up as necessary for continuity of care
  • Coordinate care with other health care providers when referrals are necessary
  • Be familiar with and follow adopted community health center standards of care
  • Maintain accurate, complete, and timely documentation regarding patient contacts within (EMR) patient charts. Charting to be done during a normal workday
  • Assess grant criteria and incorporate into the patient encounter
  • Consistent attendance and punctuality are required and expected to work the hours set
  • Expected to complete required trainings such as CPR, Mandt, Mental Health First Aid and any others deemed necessary for this position

 

TEAM DUTIES & RESPONSIBILITIES:

 

  • Assume a leadership role for a primary care team
  • Assist in facilitating coordination of care
  • Develop and implement a plan of care and recommends interventions to attain optimal patient function
  • Effectively utilize electronic health record (EHR) to document clinical activity and manage patient population
  • Work effectively and cooperatively with other members of Care Team

Participate in Care Team training and learning activities
