Schedule & Work Expectations

Monday–Friday, 8:00 AM – 4:30 PM (no nights)

Key Responsibilities
-
Deliver comprehensive primary care
-
Diagnose and manage acute and chronic conditions (e.g., diabetes, hypertension, COPD)

Perform preventive screenings and routine health maintenance

Conduct mental health screenings and coordinate specialty referrals

Lead care coordination within the PACT team model
-
Educate patients on lifestyle modifications and disease prevention strategies

Address geriatric care needs, including fall risk and cognitive concerns
-
Provide care through both in-person and telehealth modalities
-
Maintain timely and accurate documentation in the VA electronic health record (EHR)
-
Review inpatient admissions and ensure proper clinical handoffs

Adhere to all VA policies related to patient safety, infection control, and compliance

Participate in staff meetings, peer reviews, and quality improvement initiatives
Required Qualifications

Active, unrestricted U.S. Physician License (any state license honored)

Board Certified or Board Eligible in Internal Medicine OR Family Practice

Eligibility for and successful completion of VA credentialing and privileging

Strong clinical judgment, communication, and documentation skills

Proficiency in written and spoken English
